WPATH Hormone & Surgery Readiness Assessment
Gender-affirming care is a thoughtful, compassionate approach to supporting and respecting an individual’s gender identity, with the aim of providing a sense of value, understanding, and validation. This care extends beyond medical interventions such as hormone therapy or surgery, it includes using the correct name and pronouns, creating safe spaces, and ensuring you feel acknowledged and heard. The overall goal is to empower you to feel authentic and true to yourself.
To prepare for a hormone-therapy readiness assessment, you’ll meet with a clinician trained in the World Professional Association for Transgender Health (WPATH) guidelines. These sessions explore your intentions for pursuing medical gender affirmation and help affirm your gender identity. The assessment follows the WPATH Standards of Care, Version 8, and typically covers personal and family medical history (physical and psychological), your social and financial support systems, and your individual gender identity. We also discuss potential risks and managing expectations of care.
WPATH Hormone & Surgery Readiness Assessment
We do not seek a specific narrative there is no single correct path to transitioning. The number of sessions varies based on the complexity of what’s discussed; generally, clients meet with a psychologist for two to three sessions. Afterward, the clinician will help affirm that you’re prepared to begin feminizing or masculinizing hormone therapy, or able to provide informed consent for gender-affirming surgery. If you meet the criteria, you’ll receive a letter of support, reviewed with you at a final session.
For hormone readiness, a single letter from a medical or mental health professional is all that’s needed. For surgery readiness, you’ll need a letter from a physician specialist in addition to a letter of endorsement from a WPATH-trained mental health provider.
WPATH Hormone & Surgery Readiness Assessment
Per WPATH guidelines, hormone readiness assessments are limited to individuals who have started puberty, and surgery assessments are completed only with individuals over 18. Parental participation is required for those under 16, and encouraged but not required for ages 16–18. Family involvement whether legal or chosen family is also encouraged for those 18 and up.
At times, we may identify barriers that could affect access to gender-affirming medical care (for example, unmanaged physical or mental health concerns, instability in living situations, or a lack of aftercare support). In these cases, our clinician can support you through affirmative counselling, case management, and collaboration with other professionals. We also offer general gender-affirming care and counselling throughout medical and social transition, and we have connections with other gender-affirming providers including estheticians, hair stylists and barbers, personal shoppers, speech-language therapists, endocrinologists, and pediatricians to provide wraparound, comprehensive care.

The fee for gender-affirming care testing is $225 per session. The number of sessions needed depends on case complexity.
